Financial Aid Documents
The documents listed below may be mailed to the financial aid office, or scanned and emailed to financialaid@rcbc.edu. It is acceptable to send a picture of the document as long as it is legible.
It will take approximately four weeks to review the information that you submit and award financial aid. Please be aware that additional information or clarification may be requested.
2022-2023 Academic Year Forms (Fall 2022, Spring 2023, Summer 2023)
RCBC Summer 2023 Pell Grant Request Form | This form is to be used for those students requesting to use their Pell 2 for the Summer 2023 Term(s). |
RCBC Financial Aid Summer 2023 Federal Direct Loan Funding Request | In addition to completing the RCBC Direct Loan Request Form for Federal Direct Student Loans, you are also required to complete Entrance Counseling and the Master promissory Note. Both can be found at https://studentaid.gov/. Click on the "In School" tab and scroll down to Popular Topics. Students will need their Federal Student Aid (FAFSA) username and password to login to their accounts. This information will be sent directly to RCBC within 7-10 days. How Much Can I Borrow? 2022-2023 Aggregate Verification Worksheet | The Department of Education has selected your file for a review process called verification. Financial aid eligibility will be determined after the list of missing information has been submitted and reviewed by our office. Before awarding Financial Aid, we are required to confirm the accuracy of the information you reported on your FAFSA. |

2022-2023 Custom Verification Worksheet | The Department of Education has selected your file for a review process called verification. Financial aid eligibility will be determined after the list of missing information has been submitted and reviewed by our office. Before awarding Financial Aid, we are required to confirm the accuracy of the information you reported on your FAFSA. |
2022-2023 Loan Discharge Information | If you received a Total and Permanent Disability (TPD) discharge based on VA documentation, SSA documentation, or a physician’s certification, and you wish to receive student loans while enrolled, you must • Submit a letter to RCBC from a physician stating that you are once again able to engage in substantial gainful activity, and • Complete this form acknowledging that you can’t receive a TPD discharge of the new loans based on a disabling condition that already exists when you receive the new loan, unless that condition substantially deteriorates in the future. |
2022-2023 Low Income Statement | This form is required when the income reported appears to be insufficient to support the household size. This information must be verified prior to financial aid being awarded. |
2022-2023 Special Condition Income Reduction Form | Special circumstances can exist that warrant a reconsideration of your financial aid eligibility. This can include but is not limited to income and/or marital status changes. |

Dependency Override Request Form | This form is completed by students who are deemed dependents however have an extraordinary circumstance where they should be considered independent for financial aid purposes. The US Department of Education has given guidance regarding situations that do and do not qualify as unusual circumstances that merit a dependency override. In particular, the following circumstances do not merit a dependency override, either alone or in combination: Parents refuse to contribute to the student’s education; Parents are unwilling to provide information on the application or for verification; Parents do not claim the student as a dependent for income tax purposes; Student demonstrates total self-sufficiency. Homeschool Clarification Form | This form should be completed by graduates who were home-schooled to certify that their completion is equivalent to a state high school diploma. |
Marital Status Worksheet | This form is required when there is a conflict of information on the FAFSA form or with the documentation received for verification purposes. Please complete the document in full and provide the requested information. |
Never Attended (NA) Grade - Proof of Attending Form | This form should be completed when a student has been reported as having not attended a registered class. The form gives the student the opportunity to provide information that substantiates their attendance. Failure to attend a registered course in any term may impact their financial aid awards for that term. |

Requesting Adjustment to Federal Student Loan Award |

Satisfactory Academic Progress (SAP) Appeal Academic Plan | In order to be considered, Satisfactory Academic Progress (SAP) appeals must be accompanied by a SAP Academic Plan. This form outlines the requirements a student must agree to, must be completed by the student and an academic advisor, and submitted along with the SAP appeal form. |
Satisfactory Academic Progress (SAP) Appeal | This form, ALL documentation and a completed SAP Appeal Academic Plan Form need to be received by the Financial Aid Office by November 1, 2022 for Fall Semester consideration, April 1, 2023 for the Spring 2023 semester, and July 1, 2023 for the Summer semester. When submitting this form, you explain each term where the following grades occur: D, F, I, W, AW, NC, SR, ST and X. In addition, you must provide documentation that supports the statements in your appeal. |
Student Housing Clarification Form | This form should be completed when a student has indicated on the FAFSA that they are a homeless and/or unaccompanied youth. If the student has previously been certified by a school official or homeless liaison that information can be submitted in lieu of this form. |
